Step 1: Complete the form below and press the "Submit" button. When all the required data has been entered, a verification page with a summary of your input will appear.

Step 2: Check and correct data  in the verification page. Follow the "back" button of your browser in order to your original web form to make any changes.

Step 3: Print the form and fill up the credit card information for payment. After you print the form, press the "The above information is correct. Finalize ..." button only when all the data is correct, and press it only once. At this point, your information will be entered in our database.

Step 4: Fax the credit card information with a subject "CGO Registration" to "Yan Solihin" at (919) 513-7075.

Step 5 : Make sure that you book the hotel by Feb 28 to get the low rate.
